This is to be filled out by a person that has been in a ministry position and has observed you as a leader in that ministry such as a pastor, FCA leader, Campus Crusade For Christ leader, etc. This should be someone who is older than you and has been in a supervisory role over you.

This is to be filled out by a person that is a peer such as a roommate, friend from camp, etc. This should be someone who is your age and could accurately evaluate your spiritual life in regards to consistency and depth.
